What is aftercare?
Aftercare is an outpatient service that helps you successfully manage the transition back to everyday life after your rehabilitation. It is provided by specialized centers near you and is free of charge when your clinic submits an aftercare application.
What types of aftercare are available?
Combined Aftercare
Pairs in-person sessions with digital support through the coobi care app.
10 sessions + coobi care App and Smartwatch
- Every 2 weeks in-person
- Up to 2 sessions with reference persons
- Individual or group format
- Access to coobi care app + Garmin loaner smartwatch
- Direct support from your therapist + dashboard to track your progress
Requirement: Your rehab pension fund must be DRV Nord, DRV Braunschweig-Hannover, or DRV Bund.
Classic Aftercare
Takes place at your local aftercare center and accompanies you over 6 months with regular sessions.
20 sessions
- Weekly in-person
- Up to 2 sessions with reference persons
- Individual or group format
- No app or technology required
- Direct support from your therapist
Available with all pension insurance carriers.
How to find your aftercare

- 4Your rehab team submits the aftercare application. For combined aftercare, a recommendation letter is also needed.
Frequently Asked Questions
Is aftercare free of charge?
Yes, participation in both classic and combined aftercare is free of charge. Costs are covered by the pension insurance provider.
Can aftercare be extended?
Yes, aftercare can be extended by another six months if needed. The application is submitted by the aftercare center.
Can aftercare take place by phone or video conference?
No, both combined and classic aftercare are only available in person.
What do I need to participate in aftercare?
To participate in aftercare, you need an aftercare application from your rehabilitation clinic, which must be confirmed by your pension insurance provider. For combined aftercare, you also need a recommendation letter for combined aftercare.
Where do I get the recommendation letter for combined aftercare?
The recommendation letter is issued by your therapy team at the rehab clinic. You can download the template here and give it to your team.
