As a rehab clinic, you play a central role in connecting your patients with aftercare.
The role of the rehab clinic
The rehab clinic informs patients about aftercare options, supports them in finding a suitable center, and submits the aftercare application to the pension insurance provider.
The referral process

- 3Submit the aftercare application to the responsible pension insurance provider.
- 4For combined aftercare: Issue an additional recommendation letter.
Combined aftercare at your clinic
Combined aftercare is a pilot project, funded by DRV Nord, that pairs in-person sessions with digital support. Participating pension insurance providers are DRV Nord, DRV Braunschweig-Hannover, and DRV Bund. As a clinic, you can actively inform your patients about this option.
Classic vs. Combined Aftercare
Classic Aftercare
20 sessions over 6 months, weekly in-person. Individual or group format, plus up to 2 sessions with family members.
Combined Aftercare
10 sessions every 2 weeks, supplemented by digital support via the coobi care app and a Garmin smartwatch.
Requirements
Combined aftercare is only available at participating centers. Additionally, the patient's pension insurance provider must be DRV Nord, DRV Braunschweig-Hannover, or DRV Bund.
Important Documents
An aftercare application is required for aftercare. For combined aftercare, an additional recommendation letter must be issued.
