Aftercare helps you sustainably integrate what you learned during rehab into your daily routine.

Combined Aftercare

Currently available as a pilot project

Pairs in-person sessions with digital support through the coobi care app.

10 sessions

  • Every 2 weeks in-person + 2 sessions with reference persons
  • Individual or group format
  • Access to coobi care app + Garmin loaner smartwatch
  • Dashboard for your therapist

Requirement: Your rehab pension fund must be DRV Nord, DRV Braunschweig-Hannover, or DRV Bund.

Classic Aftercare

Takes place at your local aftercare center and accompanies you over 6 months with regular sessions.

20 sessions

  • Weekly in-person + 2 sessions with reference persons
  • Individual or group format

Is combined aftercare available for me?

Combined aftercare is currently available as a pilot project at selected centers. Your rehab pension fund must be DRV Nord, DRV Braunschweig-Hannover, or DRV Bund. Ask your aftercare center whether they participate in the pilot and if spots are still available.

How to participate in aftercare

  1. 1Talk to your therapy team, social services, or discharge management at your rehab clinic.
  2. 2Find a suitable aftercare center through this platform.
  3. 3Send an inquiry and schedule an appointment for your aftercare admission.
  4. 4Your therapy team just needs to submit an aftercare application. If you can imagine doing combined aftercare, your rehab team should also issue a recommendation letter. You can still decide which form of aftercare you prefer when you start.
